Step 3: Book Your Enrollment Appointment

Contact the Mill Creek Registrar by email millcreekhsregistrar@gcpsk12.org for an appointment.

Reminders:

• The enrolling parent (legal parent or guardian) and student must attend the enrollment appointment.
• No hard copies will be accepted during your enrollment appointment.
• All documents must be uploaded at least two days before your enrollment appointment.  If all documents are not uploaded, the enrollment process could be delayed.  
• The enrolling parent or legal guardian and student should arrive 5 minutes before their enrollment appointment.
• Reminder emails will be sent with details about your enrollment appointment, including the date and time. 

It is extremely important to bring a completed withdrawal packet from the GCPS school your child is withdrawing from this year.  ONLY the enrolling parent or legal guardian can withdraw the student from another school in GCPS.   

Any incomplete withdrawal packets could cause a delay in your enrollment process at Mill Creek High School.

Step 1: Withdrawal Packet, Status Change Form, Complete OLR

• Complete the OLR for the transferring student. Enrolling Parent/Legal Guardian must upload proof of residency and other required documents. - Complete at least 2 days before your enrollment appointment

Step 2: Book your Enrollment Appointment

Step 3: Enrollment Appointment Reminders

• Bring K-12 Status Change Document to your enrollment appointment.    
• OLR Required Documents Uploaded - Proof of Residency and other required documents should be uploaded 2 days before the enrollment appointment. 

Step 1: Schedule an Appointment at the International Newcomer Center

International Families:  Schedule and intake appointment with the International Newcomer Center (INC). All families enrolling from outside the United States must first visit the International Newcomer Center prior to coming to Mill Creek High School.

Step 2: Schedule an Appointment with Mill Creek High School

After completing your intake appointment with the International Newcomer Center, you must schedule a registration appointment with Mill Creek High School Registrar by email at millcreekhsregistrar@gcpsk12.org.

STEP 3: Email Reminders for Enrollment Appointment

Reminder emails will be sent with details about your enrollment appointment, including the date and time. ​

Step 4: Mill Creek High School Enrollment appointment

The enrolling parent or legal guardian and the student should arrive 5 minutes before the scheduled appointment. All documents should be uploaded no later than 2 days before your appointment. The list can be found here: Required GCPS Documents for New Student Registration.

Guardianship and Enrollment:

To register a student, you must be the parent or legal guardian. You must have legal guardianship papers from the courts or a copy of the student's birth certificate and your ID/driver's license/passport, matching the name(s) on the documentation.

The following persons are authorized to enroll students:

• Parent (natural or adoptive)
• Legal guardian
• Grandparent with a properly executed power of attorney for the care of a minor child
• Kinship caregiver with a properly executed Kinship Caregiver Affidavit
• Foster parent appointed by a state agency
• Sponsor for approved international Exchange Program

The person authorized to enroll should present one of the following:

• Driver's license
• State identification card
• Passport

Present one of the following:

• A certified copy of a birth certificate OR
• A federal, state, county, or school document with date of birth.

Examples include a certified hospital-issued birth record or birth certificate, military ID, valid driver's license, passport, adoption record, religious record, signed by an authorized religious official; official school transcript, official immigration documentation; or affidavit of age sworn by parent/guardian or other authorized person, accompanied by a certificate of age signed by a licensed, practicing physician which states the physician has examined the child and believes the age as stated in the affidavit, is substantially correct.

GCPS requires one of the following from Column A and one from Column B to show that a family lives in the attendance zone.

Column A:
Present one of the following (must include address)

• Non-contingent sales contract
• Current lease/rental agreement
• Current residential property tax statement or bill
• Current warranty or quit claim deed
• Current home purchase agreement
• Current homeowner's insurance policy

AND

Column B:
Also present one of the following (must include address):

• Current water bill
• Current electric bill

OR

• Business mail
• Renter's insurance
• Bank statement
• Car insurance

A residency affidavit is needed if the student's family is residing in the home or apartment of another individual. 

The following are necessary for enrollment:

1. The individual with whom the student's parents/guardian is residing must accompany the parent to the school to provide identification, and proof of residency.

2. Notarized third-person affidavit of residency (availabie at school) including:
a. Signature of person with whom the family is living.
b. Signature of parent/legal guardian or student, and
c. Apartment manager's signature (if applicable)

3. Two forms of Proof of Residency for person with whom family is living: See Proof of Residency section above, present one from each column)

4. One piece of business mail in the parent/legal guardian's name with the address of the home or apartment in which the student is residing. (This must be provided within 30 days or enrollment.)

One of the following immunization forms are required for enrollment:

• GA Health Department Form #3231 (Certificate of Immunization)

OR

• A notarized Affidavit of Religious Exemption (GA Department of Health Form #2208)

The certificate of immunization must be completed by the health department or your healthcare provider.  For the #3231 form to be valid, it must be marked with either " Date of Expiration" or as "Complete for School Attendance."
(A certificate marked with a "Date of Expiration" expires on the date indicated and a current certificate must be submitted within 30 days of expiration.) A medical exemption, if applicable, should be noted on a Form #3231 with a current date of expiration.

Enrolling students must also provide:

• GA Health Department Form #3300: Vision, Hearing, Dental, and Nutrition Screening.

This form is available from the health department or your doctor/dentist.  Letters from appropriate healthcare professionals and out of state certificates are acceptable, if completed within the last 12 months and stapled to the state form.
